Gabriel Guerrero started the comeback with a three-run homer in the seventh to get the 'Cakes on the board, and the momentum carried over to the following inning. JT Riddle lifted a sacrifice fly to bring New Orleans within 7-4, and two batters later, Harold Ramirez launched a three-run blast to tie the game.
New Orleans briefly grabbed a lead courtesy of Monte Harrison's two-out solo homer in the top of the ninth, only for Round Rock to answer in the bottom half on Kyle Tucker's leadoff home run, evening the score at 8-8.
In the extra frame, JT Riddle singled to put runners at the corners before Austin Dean ripped the Baby Cakes' fourth home run in four innings, a third three-run shot to provide the winning margin.
R.J. Alvarez, whose 24 saves for the Express last season led the league, retired Round Rock in order in the bottom of the 10th to preserve the biggest New Orleans come-from-behind victory since at least 2001.
Round Rock took advantage of three walks by 'Cakes starter Ben Meyer in the second inning to grab a 1-0 lead, then tacked on three more runs in the third, capped by Nick Tanielu's two-run single.
Jack Mayfield belted a three-run homer off Mike Kickham in the sixth to give Round Rock its largest lead at 7-0.
Ramirez and Dean combined for seven of the Baby Cakes' 12 hits, totaling three doubles and two home runs with six RBI. Dean went 3-for-4 to stretch his hitting streak to 10 games to open this season and 16 games dating back to last season, while Ramirez posted the club's first four-hit effort of the year.
Despite surrendering Tucker's home run in the ninth for his first blown save, Alvarez (1-1) was credited with the victory by tossing the final two innings of relief for a 'Cakes bullpen that logged 6 2/3 innings. Meyer was charged with four runs on five hits in 3 1/3 innings, walked five and struck out two.
Kent Emmanuel (1-1) took the loss after Express starter Ryan Hartman fanned seven in five shutout frames.
The teams continue the five-game set on Thursday at 7:05 p.m.
